What is Circumplast®?

Circumplast® is a plastic single-use sterile device which comes with a ligature. The device is sterilised with Ethylene Oxide and is available in four different sizes. The size range is 9.5 mm, 11 mm, 12 mm and 13 mm. The size is for the inner diameter of the device, which is inserted over the glans of the penis.

Circumplast® vs Plastibell Device

Circumplast® device is a cylindrical design which eliminates the migration/impaction of the ring to the shaft of the penis. In comparison, the Plastibell device has many well-known complications associated with it.

Features of Circumplast® For Circumcision

  • During the procedure, the glans penis is kept entirely safe as the device prevents direct contact between the external meatal opening and modern-day nappies. This feature helps to prevent meatal stenosis.

  • Circumplast® has a unique design which allows ligature to be placed in multiple locations. It gives the surgeon the option to change the position or location of the ligature, if placed incorrectly or if the amount of inner mucosal membrane secured is insufficient, before the cutting of the foreskin. 

  • The result of this advanced design allows the surgeon to choose the amount of foreskin to be removed by securing the ligature at any location desired.

  • The proximal lip of Circumplast® allows the device to cut the mucosal skin near the coronal groove. This also stops any future adhesions of the inner mucosal skin to the penile glans and provides a superior cosmetic appearance.

  • The Circumplast® handle only attaches on one side, allowing the surgeon to see the glans and external meatal opening through the device. After the circumcision, cleaning with water is easy and safe as the water can enter the ring. The glans penis is at a distance from the site of the handle detachment.

CIRCUMPLAST VS PLASTIBELL CIRCUMCISION

Circumplast® device is a cylindrical design which eliminates the migration/impaction of the ring to the shaft of the penis. In comparison, the Plastibell device has many well-known complications associated with it.

Distal Opening

The Circumplast® device has a cylindrical shape, making the distal opening of the device the same diameter as the proximal opening. This feature ensures that the glans cannot be blocked in any way and eliminates any danger to the glans.

The Plastibell circumcision device comes in a bell shape, so the distal opening has a smaller diameter than the proximal opening. The role of the distal opening is devised to act as a blockade to the penile glans. However, this has created complications such as proximal migration of the Plastibell device. This happens when the glans slip through the distal opening, which can lead to ischemia of the glans and possibly necrosis.

Location of Ligature

The Circumplast® circumcision device ligature can be secured at several locations and can also be used to select the amount of foreskin which needs to be removed. These locations are provided within the ribs of the device for the ligature to be secured. The rib ensures that the ligature cannot migrate by acting as a static barrier. The Circumplast® circumcision device has an important feature which enables it to be inserted up to the coronal sulcus. This secures the ligature along the body of the Circumplast® device without inducing any tension in the foreskin. The combination of rib and distal opening completely removes the possibility and threat of injury due to proximal migration.

The Plastibell circumcision device only provides one location to secure the ligature. The device governs the amount of foreskin to be removed, rather than the surgeon. To remove more significant amounts of foreskin, the surgeon must pull the foreskin over the device and secure the ligature once the required level is reached. This method introduces tension into the foreskin, ultimately causing the foreskin to pull the distal opening of the Plastibell device over the glans, causing proximal migration. This can cause injury to the glans and the shaft of the penis, leading to the glans' necrosis. 

Handle of Ring

The Circumplast® device handle guarantees that there is no contact with the glans. The position of the handle simplifies the location of the forceps which the surgeon places to hold the foreskin in place during the securing of the ligature. To simplify the procedure, the surgeon can place the forceps at any location along the circumference of the distal lip or handle.

The Plastibell handle is designed to provide one location to secure the forceps which hold the foreskin in place when securing the ligature. The surgeon must place the forceps on the handle as there are no alternative locations.

Proximal Lip of Circumplast

The Circumplast® circumcision device has a unique proximal lip located circumferentially at the proximal opening. This lip has several functions. First, the lip acts as a visual indicator of the location of the proximal end of the device and can be seen through the foreskin. The lip also provides a location at which the maximum amount of foreskin can be removed without inducing tension into the foreskin.
